We ranked 20 health & fitness sub-niches by estimated revenue and competition, using data from 46,361 iOS apps. See which fitness app ideas actually pay.

Health & Fitness is one of the App Store’s biggest categories: 46,361 iOS apps in our dataset as of July 7, 2026. Of those, 3,957 (8.5%) show any revenue signal at all, and 1,667 (3.6%) clear an estimated $1,000/month — slightly better than the 3.2% rate across the whole App Store. Among the apps that earn anything, the median is an estimated $643/month and the top 10% clear $16,714/month.

Those category-level numbers are almost useless for picking what to build, though. The real story is inside the category: the median $1k+ period-tracking app earns an estimated $14,786/month while the median $1k+ pilates app earns $2,304 — and the competition you would face differs by two orders of magnitude. So we split Health & Fitness into 20 sub-niches and measured each one.

How we measured the sub-niches

  • Sub-niche = apps whose App Store title contains the keyword (e.g. “meditat” catches meditation and meditate; “breath” catches breathwork and breathing). Crude but consistent.
  • Revenue = labeled estimates generated by GetAppNiche’s proprietary multi-signal model. Nobody outside Apple has reported per-app revenue; treat every dollar figure here as directional.
  • Competition proxy = the median rating count of apps in the sub-niche already clearing an estimated $1k/month. It answers: how big are the incumbents you would need to displace?

What the numbers actually say

The highest ceilings come with the heaviest incumbents. Period tracking has the highest median among winners — an estimated $14,786/month — and running is second at $12,214. But the median period-tracking winner carries 23,133 ratings, the most entrenched incumbent mass in the entire category. High revenue there is a description of who already won, not an invitation.

Calorie tracking is the standout ratio. An estimated $10,607/month median among $1k+ apps, with winners at just 3,098 median ratings — strong revenue against moderate competition. Sixty-eight apps clear $1k, so this is a proven market, not a fluke of one hit.

Pilates and pregnancy look like niches still being settled. Pilates has 978 apps in the bucket but only 24 clear an estimated $1k/month, and those winners have just 1,254 median ratings — young apps, not decade-old giants. Pregnancy shows the same shape (8 winners at 1,305 median ratings). Revenue medians are modest ($2,304 and $3,694), but these are the sub-niches where a new entrant is fighting peers, not institutions.

Yoga is crowded at the bottom, thin at the top. 1,048 apps, yet only 32 clear an estimated $1k/month. That’s a discouraging conversion rate — but the winners sit at only 2,940 median ratings, so the top of the niche is reachable if you can find an angle the existing apps miss.

Step counters, meditation, and water apps are hard mode. Winners in step counting and meditation carry 11,000+ median ratings — you would be fighting giants for default-app territory. Water tracking is worse on both axes: winners at 13,000 median ratings and only 14 apps clearing $1k, while the adjacent hydration bucket has just 2 earners at an estimated $1,607/month. Small and weak.

Posture is a genuine unknown. One app clears an estimated $1k/month, and it has only 95 ratings. That’s either an empty niche waiting for a serious entrant or a niche with no willingness to pay — this table cannot tell you which. The only honest next step is checking demand directly: keyword difficulty and search volume signals, which take minutes to pull (see below).

How to validate one of these in 30 minutes

  1. Search the sub-niche in the free revenue checker. Type “pilates” or “calorie” into getappniche.com/top-grossing-ios-apps — free — and open the top results.
  2. Check the estimated revenue of the leaders. Do several apps show real money, or does one giant own everything? A niche with 10 mid-sized earners beats a niche with one whale.
  3. Score 2–3 keywords in the free App Store keyword tool. You want evidence of search demand with difficulty you can realistically beat.
  4. Compare against the baseline odds. Across the whole App Store, 3.2% of apps clear an estimated $1k/month — but 13.76% of freemium apps do, versus 1.8% of free apps. Your monetization model shifts the odds more than most niche choices.

Honest caveats

  • Title-keyword bucketing is approximate. A pilates app without “pilates” in its title is missed; a “Yoga & Pilates” app counts in both buckets. Treat the table as a map, not a census.
  • Every revenue figure is a labeled model estimate. Nothing here is audited, and individual apps can be off.
  • This is a snapshot from July 7, 2026. Sub-niches move; pilates today may look like yoga in three years.
  • A high median among winners can mean “a few winners”, not “easy money”. Period tracking’s $14,786 median describes 24 apps that already survived a brutal market — it says nothing about app #25’s odds.
